Purse String Breast Reconstruction

Before After Case 1. Patient with large left breast and sagging right breast after attempt at breast augmentation alone. Result at 6 weeks from purse string left reduction with augmentation, and purse string right lift with augmentation.

Before After Case 2. Patient with previous prophylactic mastectomy with purse string method with outside attempt for revision leading to asymmetry with upriding right implant and high nipple position. Result at 2 months following repeat purse string bilateral lift with implant repositioning.

Before After Case 3. Patient returning to outside surgeon for implant replacement and bilateral T scar lift. Unfortunately complicated by severe infection with loss of part of right breast and outer areola. Result at 1 year following reinsertion of custom implant on right, gland reshaping, and purse string areolar lift and restoration.

Before After Case 4. Patient with cystic breasts and strong family history of breast cancer, having prophylactic purse string approach mastectomy with custom expandable implants. Result at 5 months.

Before After Case 5. Patient with strong family history of breast cancer at early age, having underneath approach mastectomy with custom expandable implants. Result at 1 year.

Before After Case 6. Patient with strong family history of breast cancer and cystic breasts, having underneath approach mastectomy with custom expandable implants. Result at 3 years.

Before After Case 7. Patient with previous prophylactic mastectomy with implants over the muscle, undergoing revision with submuscular replacement and desired larger cup size. Result at 4 years.

Before After Case 8. Young patient presenting with total right mastectomy. Had right implant reconstruction with left T scar augmentation-lift. Returned for removal of silicone implants and secondary reconstruction with right PEG (periareolar flap) TRAM abdominal flap plus repeat secondary left T scar lift. Result at 5 months prior to nipple creation on right. Currently Peg flap is done more commonly with skin sparing modern mastectomy, and Purse string breast lift may replace T scar and Vertical scar breast lift.
