         We appreciate the opportunity to provide information on options for buttock
rejuvenation following massive weight loss as a result of your research and
referral from the ASPS Plastic Surgeon referral center.   To begin with, as
you know from your research of the Brazilian buttock lift, it is composed
principally of two procedures, (1) one is the removal of fat around the
buttocks to better frame and highlight the buttocks which can often appear
hidden in a perimeter of fat that occurs in the hips, lower back, and outer
thighs, and (2) second, the processing of that fat for structured
micro-injection into the substance of each buttocks cheeks to augment the
contours. Usually 200 to 400 cc of fat must be available to secure a
reasonable change in buttock contours.  Some of this fat will be absorbed
over time, and one cannot be absolutely sure of the amount of fullness that
will remain at 2-5 years, even in the best of hands.

         Since you have shed 100 pounds, it would seem that you might not have the
reserves of fat in the surrounding donor areas which can include the hips,
abdomen, flanks, lower back and thighs, etc to add volume and provide a
significant lifting to the buttocks. Patients who present with sagging
buttocks as a result of significant weight loss are usually devoid of
contour-filling fat and consequently need surgical lifting. This is similar
to the breast lifting one does with breast sagging, and tummy tuck for
abdominal sagging after weight loss and pregnancy.  The Brazilian buttock
lift by definition is a misnomer in that it is a reshaping of the buttocks
which depends upon a tight skin envelope, not one that is loose and unwieldy
after massive weight reduction.  If your skin has come down nicely and there
is sufficient fat under firm inelastic skin, then fat may be transferred a
la Brazilian methodology.

         I agree that buttock implants while popular in some quarters and address
filling of loose skin, and are an immediate solution to the loss of buttock
fat, (as with weight loss),  but have a few more tradeoffs over time.  The
side effects and potential complications are higher with buttock prostheses
than with breast prostheses.  The preferred method of buttock augmentation
is the use of soft gel round implants that are seated under the muscle. They
must be custom ordered after making measurements on the patient.  These
implants are more expensive than breast implants.  If they remain soft, they
will retain a slightly round full shape of the preferred full round buttocks
sought by many people.  I do not endorse buttock implants in the majority of
patients such as yourself, but advise liposuction and occasionally
Brazilian-type transfer of that fat to the buttocks.

Some small thin women do not have enough fat on their body in order to
perform the Brazilian butt lift procedure. Thus, if a woman does not have
enough donor fat, the other option would be butt augmentation with silicon
implants. For butt implants, a small incision is made in the crack between
the buttock cheeks. The butt implants are then carefully placed in the
tissues of the buttock avoiding the areas where important nerves lie. When
the incision heals it is difficult to detect. The result is a round and
prominent butt.

          The patient who has undergone massive weight loss presents several problems
for the plastic surgeon, in that the skin glove is loose in a 360 degree
geometry. Often, simply addressing the buttocks alone may rejuvenate one
area and leave the adjacent areas untouched and then they may  become the
source of displeasure and concern. It is like tailoring slacks for a loose
buttocks, and then one sees adjustments in the outer thighs and inner thighs
that need tailoring.

         Thus options for care may include: fat transfer from the surrounding areas
with liposelection injection: (Br Butt L)

                                                   :buttock and lower body
lift
                                                   :saddle lift of the
buttocks where a wedge is removed from the center of cheeks

Dr. Hickman:
I have seen "butt lift" procedure on cosmeticsurg.net, where a Dr. Rodgriguez in Baltimore performs this surgery.  Is this something that you are able to perform?   What I am interested in is the lifting and removal of sagging or excesss skin.  I am a fairly thin, very fit person, but heredity has given me a rear that sags a bit and has some appearance of cellulite.  I feel that the lifting of the buttocks is something that will provide a more youthful appearance.  Will this type of procedure help?
Also what are the costs involved in this type of procedure?

Thank you for your comments and questions. I have responded directly to your email.  The surgical lift may be indicated where there is excess skin and loss of superficial fascial system support which can occur during natural aging, even in fit individuals.  The operations as a rule can be outpatient procedures where general anesthesia is used.  Fee structure is based on extent of lifting and ancillary measures to optimize the lift at the same time.  Recoveries are usually less than a week for general activity and driving.  Longevity of results depends on the elasticity or inherent stretchiness of skin that is removed and tightened.  We perform an array of lifting procedures for the torso, upper and lower extremities.
