There are perhaps 75 injectables used worldwide to enhance lips, reduce nasolabial folds, and contour cheeks and chin.   Only a few are available in the United States and most continue to require long term studies to determine safety and effectiveness.  Clinical trials permit new products to reach the market as studies are completed.
 
At present,  fat and the patient's own tissues appear safest and require no allergy skin test. A site for collection and a site for placement mean that two areas need to be anesthetized and the cost of these procedures is usually greater than for the many commercially available products. 
 
Cadaver products  such as Alloderm provide bulk to lips and may last 1-3 years. (Cost $2500).  Fasica-fat grafts taken from an old scar or area well hidden from view, can be transferred to plump up small lips, and can last years. These type of grafts may cost about $4900.  There is immediate and bold enhancement with these materials and can last 1-5 years.
 
The latest commercial products becoming available for off-label FDA use as soft tissue enhancers, not bulking agents!!!!!! are:  Restylane and Perlane (QMed Sweden) (non-animal based hyaluronic acid (may last 3-9 months)(Cost $675 for 0.7cc perlane and $375 for 0.4cc of perlane and restylane) ( and Cosmoplast (Inamed)(Human Collagen implant)(lasts 4 to 6 months) (Cost $500-700 for both lips).  Juvederm, the more potent hyaluronic acid filler may last 6 to 12 months (Cost: $580 to $950 for both lips). Juvederm may be more cost efficient for durability and ease of use at present. The FDA approved forms in the US are Juvederm Plus and Juvederm Ultra. Juvederm Ultra is generally preferred.

Sculptra and Radiesse is cleared for use in the HIV patient for facial atrophy may be used off label for thin and hollow cheeks in others.  Cost $2000 and upward for considerable plumping. NEITHER SUBSTANCE IS CURRENTLY RECOMMENDED, IN FACT ARE GENERALLY PROHIBITED, FOR USE IN THE HUMAN LIPS.

Radiesse pricing has been improved by the kind support of the manufacturer.  It has been used successfully for CHEEK AUGMENTATION AND CHIN AUGMENTATION in selected patients who want some enhancement but are not yet committed to permanent implants for those areas. 

Fees for  combined cheek and chin augmentation using RADIESSE can be as low as $3000 when using RADIESSE compared to  approximately $8500 for permanent implant surgery to both areas.  The material may last up to 2-3 years.  The adjustability of the pliable filler may be an advantage for some consumers.
